Appointments are available Monday through Friday 8:30 - 4:30 PM.
The day of your appointment depends on each individual Physician and
their schedules. If any appointment is not kept without a 24 hour
cancellation or reschedule notice there is a $25.00 charge, which is
the patient’s responsibility.

EMERGENCIES
|
|

Call during office hours to be scheduled as quickly as
possible.
Our physicians are available after hours, on Holidays, and on
weekends, for emergencies only. Call
our office (510) 284-4100. You can page the physician in
an emergency or leave a voice mail message for the next business day
by following the telephone message during off office hours. Our
answering service is available for back up.

PRESCRIPTIONS REFILLS
|
|

|
All Physicians request that you Obtain prescriptions at the
time of your visit.
If you need a prescription refilled in an emergency please have your
pharmacy fax your request during business hours. Prescriptions may
take up to 24 hours. All prescriptions require approval by our
physicians. We regret that we will not be able to fill prescriptions not
presented by our physicians and for patients who have completed their
surgery follow up care. Your Primary Care physician should continue
on with all your medical needs.

COMPLETION OF REQUIRED FORMS
|
|

|
|
All State Disability forms, DMV Handicap forms, and all
Required Forms for Worker's Compensation will be
completed by your physician. For all others there is a handling fee
of $30 for copies of medical records not exceeding 10 pages. All others will be charged
$50.00

GUIDELINE BILLING
|
|

Please be sure to bring your Insurance cards and a picture ID
with you to your appointment. Your card(s) are needed
to verify coverage and is the only way we will be able to bill your
Insurance. If you are unable to provide us with your current
Insurance information, we may need to cancel and reschedule your
appointment. All co-pays and deductibles, if applicable, are due
before services are rendered at check-in time.
Referral forms are required
for any HMO or Managed Care Plans. If you do not have your
Referral Form from your Primary Care Physician at check in time your
appointment will be rescheduled.
Cosmetic Consults and
surgeries not covered by Insurance must be paid prior to seeing the
physician.

SURGERY
|
|

|
When you
and Your Physician have decided to proceed with surgery, our surgery scheduler will contact your
Insurance for prior authorization. In some cases depending
on your Insurance, this can be done in one or two days and others
weeks. When this process has been completed, and according to the
urgency of your surgery, your
surgery will be scheduled and you will be notified of your date and
time. If you have any questions please do not hesitate to
call the office.
